Privacy statement

Eastern Heritage processes your personal data by using our services and / or because you provide these yourself to us.

Eastern Heritage will ask for your name, telephone number and e-mail address when making an appointment. Prior to the first treatment, an intake interview will be held, during which you will be asked for details regarding your health. This is a prerequisite to be able to tailor the treatment to your situation. Other information you will be asked to provide is your date of birth and address.

Eastern Heritage processes your personal data for the following purposes:

– In order to be able to tailor the treatment to your situation and to be able to provide a responsible and safe treatment.

– To be able to make contact in connection with making or changing an appointment.

– To handle payments.

– To be able to inform about changes to our services and products.

– To comply with legal guidelines and legal obligations.

The personal data will not be provided to third parties without your written consent, except under special circumstances in which a legal obligation must be met.

The personal details, which have been provided during the intake interview and the reporting of the treatments, will be kept on paper in a locked cupboard and closed practice space. Eastern Heritage will keep these data in archive for another 7 years after the last appointment, after which they will be destroyed.

Eastern Heritage takes the protection of your data seriously and takes appropriate measures to prevent misuse, loss, unauthorized access, unwanted disclosure and unauthorized modification. If you have the impression that your data is not properly secured or there are indications of misuse, please contact us at info@easternheritage.nl.

You have the right to access, correct or delete your personal data. In addition, you have the right to withdraw your consent to the data processing or to object to the processing of your personal data by Eastern Heritage and you have the right to data transfer. This means that you can submit a request to us to send the personal information we have about you in a computer file to you or to another organisation named by you.

You can send a request for inspection, correction, deletion, data treatying of your personal data or a request to withdraw your consent or objection to the processing of your personal data to info@easternheritage.nl.

To ensure that the request for inspection has been made by you, we ask you to enclose a copy of your proof of identity with the request. In this copy, make your passport photo, MRZ (machine readable zone, the strip with numbers at the bottom of the passport), passport number and Burgerservicenummer (BSN) black. This is to protect your privacy. We will respond to your request as soon as possible, but within four weeks.
